A patient's decision aid for vascular access placement in the emergency department.
CONCLUSION: The result from this analysis indicated that most patients visiting the emergency department are not knowledgeable about their options related to vascular access device placement. The observed increase in the average correct responses on the post-test indicates that a patient decision aid can be an effective educational tool in the emergency department. Improving shared decision-making in vascular surgery: a stepped-wedge cluster-randomised trial
For various vascular surgical disorders different treatment options are available and feasible. Hence, vascular surgery seems an area par excellence for shared decision-making (SDM), in which clinicians incorporate patients ’ preferences into the final treatment decision. However, current SDM-levels in vascular surgical outpatient clinics is below expectations. To improve this, different decision support tools (DSTs) were developed: online patient decision aids, consultation cards and decision cards.

Indocyanine green angiography aids in prediction of limb salvage in vascular trauma
In cases of severe vascular trauma, traditional means for initial vascular assessment include bedside Doppler with ankle brachial indices (ABI) and validation with arteriography. Furthermore the development of the Mangled Extremity Severity Score (MESS) assessment and more recently Lower Extremity Assessment Project (LEAP) are variations that attempt to stratify clinical variable, such as extent of skeletal/soft tissue injury, limb ischemia, shock and patient age with limb salvage outcome.1,2 While these later methods help to stratify the utility of the limb salvage and other functional outcome, they do have limited applic...

Vascular and lymphatic complications after thoracic duct cannulation
ConclusionsOf 58 patients who had attempted thoracic duct cannulation, successful access was achieved in 90% of patients. Early and delayed complications occurred in 3.4% and 4% of patients, respectively. Thoracic duct cannulation represents a highly successful technique that aids in the treatment of chyle leaks in medically complex patients.

Cutaneous nerve-conscious surgical repair of vascular access-related aneurysm assisted by anatomical ultrasonography in hemodialysis patients.
CONCLUSION: Preoperative ultrasonographic examination of the cutaneous nerve facilitated the intraoperative prevention of cutaneous nerve injury in VA-related aneurysmectomy. Nerve-sparing VA surgery assisted by preoperative nerve ultrasonography based on the understanding of topological anatomy might contribute to the reduction in postoperative neuropathy and enhance comfort in the daily life of hemodialysis patients. The patient experience of hemodialysis vascular access decision-making.
CONCLUSIONS: The study participants were minimally engaged in VA decision-making. Educational aids and shared decision-making tools are needed to empower patients to make better-informed, self-efficacious VA decisions. PMID: 33118395 [PubMed - as supplied by publisher]

Fibrinogen as a Biomarker in Vascular Disease and its Prognostic Impact During Venous and Arterial Thrombolysis
Fibrinogen is a large complex glycoprotein that is aids in the formation of blood clots. As an acute phase protein, fibrinogen has clinical application as a biomarker in the management of patient with venous and arterial disease. High levels are associated with inflammation and the risk of cardiovascular disease. During venous and arterial thrombolysis therapy, fibrinogen levels have prognostic significance regarding risk of bleeding. In this review, the application of fibrinogen as a biomarker in care of the vascular surgery patient is detailed.

Homosexual desire after AIDS: an analysis of the criteria adopted by men in the search for same sex partners
This article analyzes changes and continuities in partner search criteria by homosexual men considering the impact of the HIV-AIDS epidemic in the city of S ão Paulo, Brazil. Thus, it compares two different historical moments: between 1979 and 1981 (pre-epidemic) and 2015 to 2017 (post-epidemic). In the first period, the source incorporated 120 classified ads collected from the Lampião da Esquina newspaper and in the second period, the preferences and criteria used to search for partners were analyzed using geolocalized applications, based on 120 online profiles. The data show that that between 1979-1981 the search for â...

Evaluation of factors associated with vertical transmission of HIV-1 in fourteen years of a referral center in Southern Brazil.
CONCLUSION: It was observed a decrease in the rate of vertical transmission in recent years. According to the studied variables, is suggested that the risk factors for vertical transmission of HIV-1 were absence of antiretroviral therapy, high viral load of pregnant women and the breakthrough time greater than 4h membranes. Budd-Chiari syndrome secondary to catheter-associated inferior vena cava thrombosis.
CONCLUSION: Assessment of catheter dialysis tip location with radiological image seems to be a prudent measure after each procedure even if the tunneled dialysis catheter has been introduced with fluoroscopy image. PMID: 28355402 [PubMed - in process]

Study Examines the Effect of 24 Weeks of Statin Therapy on Systemic and Vascular Inflammation in HIV-Infected Participants on ART
“HIV-infected individuals are at increased cardiovascular disease (CVD) risk due in part to inflammation. Statins decrease inflammation in the general population, but their effect in HIV is largely unknown. … This is an on-going randomized, double-blinded, placebo-controlled trial to evaluate the effect of statins on inflammatory markers in HIV. Subjects received rosuvastatin 10 mg daily or placebo for 24 weeks. Subjects were receiving stable antiretroviral therapy with low-density lipoprotein (LDL)-cholesterol ≤130 mg/dL and evidence of heightened immune activation or inflammation. … 147 were enroll...

Newborns at high risk for brain injury: the role of the amplitude-integrated electroencephalography
CONCLUSION: The aEEG has shown to be a useful non-invasive bedside monitor that aids in evaluating brain function, background activity, and cyclicity. aEEG findings have also demonstrated good prognostic value in a group of critically ill neonates. The aEEG seizure diagnosis capability has limitations, which have been already well established. The use of neonatal brain monitoring such as aEEG was shown to give valuable information in several high-risk clinical situations.PMID:34986412 | DOI:10.1016/j.jped.2021.10.008

Improving shared decision-making in vascular surgery: a stepped-wedge cluster-randomised trial
CONCLUSION: Introducing DSTs improves the level of shared decision-making in vascular surgery, improves patient knowledge, and shifts their preference towards more non-surgical treatments. The SDM-training for clinicians and the decision aid for patients appeared the most effective means for improving SDM.TRIAL REGISTRATION: NTR6487.PMID:35483576 | DOI:10.1016/j.ejvs.2022.04.016

Internet Search Patterns of Human Immunodeficiency Virus and the Digital Divide in the Russian Federation: Infoveillance Study
Conclusions: This study contributes to the methodological literature on search patterns in public health. Based on our preliminary research, we suggest that PCA may be used to evaluate the relationship between the determinants of Internet access and searches for health problems beyond high-income countries. We believe it is in middle-income countries that search methods can make the greatest contribution to public health.
